‘State Only’ Association Awards
Award Applications will be open February 1 - March 1, 2026
Nominate colleagues to be awarded with recognitioin for outstanding efforts and achievement in youth development programs!
AWARDS BY APPLICATION
Each member of UAE4-HYDP may apply for UAE4-HYDP ‘state only association’ awards by submitting the following:
- Completed ‘state only association’ application form (pg. 2).
- write-up/description which includes specific award requirements (i.e., need, activity and outcome, why you are applying for this award, etc.) 1 page maximum.
- Appropriate samples/supplemental materials (i.e., media, newsletter, curriculum, etc.) (Samples/supplemental materials are not required for ‘Service Award’ applications.)
Virtual Program Award
May be submitted by an individual or team. In a one-page write-up identify educational need, activity and impact. Include copy of virtual lesson plan or virtual delivery method.
Innovative Program Award
May be submitted by an individual or team. In a one-page write-up state what was accomplished that created greater visibility or camaraderie for Utah State University Extension Service. Include copy of innovative lesson plan or innovative delivery method.
Expanding the 4-H Audience Award
May be submitted by an individual or team. In a one-page write-up describe target audience, identify educational need, activity and impact. Include sample of activity/supplemental materials.
Specialty Award
In a one-page write-up describe target audience, identify educational need, activity, impact, and list up to 10 members involved in the project. Include sample of activity/supplemental materials.

Faculty of the Year

Coordinator of the Year
Please click on the google form link and in one paragraph describe why you feel this person is deserving of this award. (Applicable to all Program Coordinators who actively support and participates in the work of 4-H).
Staff Assistant of the Year
Please click on the google form link and in one paragraph describe why you feel this person is deserving of this award. (Applicable to all Staff Assistants who actively support and participates in the work of 4-H).
Distinguished County Team Award
Designed to recognize outstanding efforts of Extension county office teams (two or more individuals) for demonstrating an outstanding team response to a local need or issue. Recipients of this award utilize the strengths and skills of ALL team members in providing a specific program responding to a local issue. The team must be composed of members from at least two different employment/volunteer roles within USU Extension (i.e. faculty & staff assistants, staff assistants & coordinators, faculty & coordinators).
AWARDS BY VOTE
Voting process will be conducted annually through Qualtrics.
