Living Well Self-Management Programs

Living Well with Chromic Disease, Chronic Pain, and Diabetes

LWSMP through USU Extension include Living Well with Chronic Disease, Chronic Pain, and Diabetes (three separate programs). These were developed at Stanford University and require facilitators to be trained before implementing them. Facilitator trainings in Utah are free, but have to be coordinated with master training sites. Participants meet one day per week for 2-2.5 hours over six weeks and learn various health and behavior strategies for managing their condition. Sessions include education but are interactive with brainstorms, discussions, and partner-work to encourage learning from others' lived experiences. Classes are taught in person, virtually, and telephonically.

Program Management
The Utah Department of Health and Human Services (UDHHS) oversees Living Well Programs in Utah. UDHHS supports Living Well Coordinators from community organizations who oversee trained staff to implement these programs in their communities. Gabi Murza is the Living Well Coordinator for USU Extension.
