Well Connected Communities

Utah State University Extension participated in the Well Connected Communities grant for six years, to advance a Culture of Health in conjunction with Robert Wood Johnson and 4-H. 

The partnership focused on three main elements:

  1. Designing a sustainable network structure to promote health and well-being in communities across the nation.
  2. Creating and disseminating tools for healthier communities.
  3. Launching a training curriculum for local community advocates.

While the Well Connected Communities grant has ended, our project team is committed to sharing and continuing efforts to make youth equal partners in community change. Our Youth-Led Community Needs Assessment manual is a tool that can help amplify youth voices and empower youth as equal partners in tackling health disparities.

Youth Community Needs 

The Youth Community Needs Assessment Manual will provide you with enough information to lead your own youth-led community needs assessment. While this useful, feasible, and concise manual was designed for our Utah Well Connected Communities, it can be applied across all topics and is not only for folks doing health and well-being work with youth. Cris Meier

Cris Meier, USU Extension Community Resource and Economic Development Specialist and Assistant Professor in the Department of Social Work.

As an Extension Specialist she works with rural communities to provide access to resources, assist with capacity building of residents and stakeholders, implements interventions that contribute to community development, and policy, systems, and environment changes that help to improve health disparities.
