If you are a behavioral/mental healthcare provider, here is some additional information regarding our program and what it entails.

The agricultural community faces unique challenges and may have an elevated risk of suicide.  The Utah Department of Agriculture and Food and USU Extension have partnered to create the Ag Stress Assistance Program. This partnership aims to educate, provide local resources, and help alleviate the financial barrier hindering behavioral health care access for local farmers and their families. Through a grant with the USDA NIFA Farm and Ranch Stress Assistance Network, we are able to provide $2,000 for behavioral health care services per individual in the agricultural industry until funding is no longer available.

Funding Status: Unfortunately, the grant funding for this program has all been utilized, meaning we no longer have vouchers available. That being said, we still encourage individuals to seek out help from mental/behavioral health care professionals. Please see our list of participating professionals to find a provider near you that will be a good fit. If you'd like to help us to continue providing mental health assistance you can donate here. Every dollar is greatly appreciated and goes straight to the cost of care! As we receive more funding we will open up more opportunities for assistance. 

Who is eligible?

Patients working in the agricultural industry (farmer, producer, etc) and their family members. These patients are eligible with or without insurance.                           

What type of appointments are covered?

Both In-person and virtual behavioral health care appointments with a licensed health care provider are eligible for financial coverage. Possible visit reasons include: anxiety, depression, ADD/ADHD, bipolar disorder, grief, insomnia, mood swings, panic attacks, post-traumatic stress disorder, relationship problems, stress, substance use disorders, trauma.*

How do we submit the bill for payment?

If the patient is insured, we ask that you process the insurance claim before submitting an invoice or bill (this will allow our limited funding to help more local Utah farmers). When the amount owed is identified (and a release of information is signed per individual clinic protocol) you may submit an invoice or bill directly to USU Extension at sadie.wilde@usu.edu or by calling 435-919-1324 for an over the phone payment. A payment confirmation will need to be provided for our records. We also cover co-payments. If the patient is uninsured, we will cover the total visit cost. The patient may also submit an invoice themselves after it is received.

*We are not an insurance company and do not need a diagnosis or ICD-10 code for payment. Please do not send unnecessary PHI. We fully trust the discretion and expertise of our medical providers to determine eligibility for each individual patient.
