Please fill out this form for all of your plant and pest diagnostic needs!

Utah State University Salt Lake County Extension may not be able to identify or diagnose plants, plant diseases or insects via description alone. Please attach photographs of both the diseased and healthy plant parts or of the insect, preferably alive.

You may prefer to print this form and bring samples to our office. The Master Gardener help desk is open Monday, Wednesday, and Friday from 9 am – 4 pm, but you may drop off the form and sample(s) anytime during our office hours, which are Monday – Friday, 8 am – 5 pm
