IRB Survey Statement

The Retail and Foodservice Food Safety Consortium (The Consortium) is conducting this survey under a USDA CSREES sponsored research project No. 07-01864.  The Principle Investigator is Brian A. Nummer, Ph.D., Extension Food Safety Specialist at Utah State University.  You have been asked to take part because you are a food safety professional.  Participation in this survey is entirely voluntary.  All surveys are devoid of personal identifying information.  This survey consists of ___ questions regarding your needs and opinions on retail and foodservice food safety issues and should take approximately __ minutes to complete.  There are no anticipated risks to completing this survey.  Data will be used to ultimately increase the safety of our food supply.  The Institutional Review Board at USU has reviewed and approved this survey and research study.  If you have any questions or concerns, you may contact USU - IRB at (435) 797-1821 or the PI at (435) 797-2116 / briann@ext.usu.edu.
