To register for the Food Safety Manager's Certification Course, fill out this registration form

The registration form will tell you where to send the filled out form and payment (cost information will also be included on the form). Once the form is completed and sent, someone will contact you through email.

When you are ready to take the exam, call the Extension office at (435) 738-1140.
