To register for the Food Safety Manager's Certification Course, fill out the registration form. The Form includes directions about how much it costs, and where to send the form and the payment method. Once you send in the form you will receive an email with your username and password. When you are ready to take the exam, call our office, at (435) 781-5452. If you do not have access to a computer please come in our office at the Uintah County building on the 3rd floor USU Extension office.
